CBAM Clinic Patients Terms and Conditions:
Please find below the Canadian Board of Aesthetic Medicine and/or CBAM Clinic (hereinafter as “CBAM Clinic”) terms and conditions related to the CBAM Clinic Patients..
Much effort has been taken to ensure the safety and advisability of the information presented and applied in all the cosmetic and aesthetic clinic sessions presented by the CBAM Clinic. All procedures performed are within the standards set out by the College of Physicians and the College of Nurses of the local province. With any medical procedure, there is always a risk of injury when performing any type of intervention. The risk of injury increases when a procedure is performed without judicious precautions, negligence on the providers’ behalf, and/or when the recipient of the intervention has unique biological or other factors that make them more vulnerable to negative reactions. Additionally, it is possible for clients to unexpectedly have an adverse reaction where their overall health becomes compromised during or after completion of their treatment.
By agreeing to be a CBAM Clinic Patient, you certify to the following attestation:
You, as a cosmetic patient, are totally aware that you are in a Training facility and the trainees and service providers are healthcare professionals or health care professional students who are supervised directly or indirectly by a qualified instructor(s) during the interview, assessment and injection (treatment). As these are training courses or practising sessions, every trainee will train at a different speed. This means that the waiting times for you can be slightly longer than expected.
Due to the clinical environment, we strongly advise you not to bring children to your appointment. We understand you may want to bring someone else with you to your appointment but we do ask that this is kept to a minimum. If you do bring someone with you, please note they are not allowed to go into the treatment room with you. We would also ask patients to respect the privacy of others during the course of treatment.
Due to unforeseen situations, we may need to cancel or change your appointment occasionally. You will be advised of this and an alternative appointment will be offered on another day and time. If the cancellation is initiated by the CBAM Clinic, you are entitled to receive the full refund of your paid deposit, if you do not wish to change the date of your appointment to another date. In these circumstances, CBAM Clinic is not liable for any loss or damages that have occurred due to the cancellation of appointment.
Prior to the treatment, the service providers will verbally discuss with you any risks, alternative treatments, and adverse effects which can potentially occur as a result of the treatment. By accepting to move forward with the treatment, you are confirming you are aware of the risks and benefits, potential contraindications, and adverse effects which can arise from treatment. Please be aware if these instances do occur, CBAM Clinic, medical directors, staff and supervisors do have the facilities to treat and deal with adverse effects but do not take legal or personal liability.
You hereby release the CBAM Clinic, and other associated instructors, faculties and staff from any and all liability of any kind, directly or indirectly related to your treatment.
Cosmetic products brands: The products brands used on the treatment sessions are variable due to available inventory and also the market availability. We can not guarantee the availability of any products during the appointment booking and during the treatment, even if it is advertised previously. All the products are Health Canada approved and the patient is allowed to reject receiving the treatment by a specific brand or type of a product. In the case if the client rejects receiving the treatment before starting the procedure, the appointment is cancelled and any paid deposit is fully refunded. If a client cancels the treatment, due to any reason, the CBAM Clinic is not liable for any loss or damages that may have occurred due to the appointment. After starting the procedure the treatment fees are not fully or partially refundable.
Promotional Pricing: Any promotions and discounts applied to a specific date, is not applicable to other dates, locations and treatments. The promotions can not be combined together for one individual.
The prices or packages which are offered on the website or all the other platforms may change at any time without a notice. The most up-to-date price of the services or the packages are confirmed with the patient before receiving the treatment and the patient is allowed to refuse the offered price or package. In this case, the appointment of the patient is cancelled and any paid deposit will be fully refunded. If a client cancels the treatment, due to any reason, the CBAM Clinic is not liable for any loss or damages that may have occurred due to the appointment.
You agree that all videos and images taken during training sessions (which you can be in any of the images or videos) can be used or shared by the CBAM Clinic for educational and marketing purposes for the benefit of other healthcare practitioners in the field. Should you not want to share your images or videos, please mention this to the staff on the day of the treatment.
Before & After Photos:
We are required to take “before and after photos” of your treatment to keep on your file as a part of your medical information. You have complete control over what happens to these photos. We will not share these photos with any third party without your consent. (On the other hand if you are comfortable with sharing your results with others, you can have a collaboration with CBAM Clinic and receive a $25 cashback for each before and after picture!) (One before and one after photo of yourself)
When booking your appointment (new or returning appointments), we require you to pay a $50 deposit. This amount will be deducted from the total cost of the treatment. The total cost of the treatment must be paid on the day of the treatment by debit, credit card or cash. To see the fees, please refer to the confirmation email that has been sent to you by CBAM Clinic. Any payments made for the goods and services provided to the patient are non-refundable.
If you cancel or change your appointment at least 72 hours before your treatment session, we can transfer your deposit to another appointment or give you a full refund. If you cancel your appointment within 72 hours of the treatment session, or if you do not show up for the session, your deposit will be lost.
Refund policy: As the result of each cosmetic procedure is a subjective matter and it could be variable for each patient (different bodies might have different reactions to the treatments) when the product is used for you, you will be charged accordingly, and we cannot guarantee any of the treatments to fully work. (This also has been mentioned in the consent form that you sign before receiving any treatments.).
You must provide accurate up-to-date information including your medical history on each of your appointments. We will not be held responsible for any incorrect information given.
Follow up Appointments/Treatments:
If you feel you need a follow up treatment or visit for any reason, please contact 1-866-366-2226(CBAM) or email us at firstname.lastname@example.org.
In most cases, we will recommend you to let the results of your treatment settle for about 2 weeks and we will arrange a follow up appointment with you between 2 and 3 weeks after the treatment. In some cases, the follow up appointment may not be in the original place of treatment and you may need to travel to a different clinic where your direct supervisor practices for the follow up appointment. Please note all the services and products which have been used during the follow up treatments will be charged as the current price list or available promotions of the CBAM Clinic on the date of the follow up visit.
Treatments Follow up policy:
As majority of the aesthetic treatments such as injection of Neuromodulators and fillers need a period f time to demonstrate the maximum results, we recommend the patient to wait at least 2 weeks before booking for a follow up session (the follow up session can be skipped if the client is fully satisfied and has no specific concern).
If any follow up visit is arranged, during the session, the result of the previous treatment(s) will be discussed with the client and will be tried to maximize the result of the procedure with or without doing minor corrections
As for many of the clients, the amount of the products injected to create a full satisfactory result is not predictable in only one session, in case of using any products for the corrections during the follow up sessions, the client will be responsible for the cost of the procedure. There will be no free of charge service in the follow up sessions. As the result or longevity of aesthetic procedures are multifactorial and unpredictable, we can not guarantee the result but we try to maximize the satisfaction of our clients. There is absolute no refund policy in case of not satisfaction with the results or the result longevity.
Note: For HA fillers, any dissolving of the product should be discussed and approved by the medical director and/or the injector and can not be initiated only based on the client’s request, as many of the complications or unsatisfactory results can be managed without using the dissolving agents. In any case, the client is responsible for the service fee of the dissolving agent. Also the previously used filler for the treatment is not refundable.
